ASM Medical Release Form

This PDF download must be filled out and given to the church office or youth staff if your child plans on attending a trip or event with Agape Student Ministry.  Must be completed and turned in or on file for your child to be able to attend trip/event.  Note that the form must be notarized and will be valid for one complete year.
